Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that facilitates research into pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2, permitting multiple and varied meta-epidemiological studies that evaluate the effect of treatment on trials that employ different levels of pragmatism, as well as other design features.
Background
Pragmatic trials are becoming more widely recognized as providing real-world evidence for clinical decision-making. However, the use of the term "pragmatic" is not consistent and its definition as well as assessment requires further clarification. Pragmatic trials are designed to inform clinical practices and policy choices, rather than confirm a physiological hypothesis or clinical hypothesis. A pragmatic trial should aim to be as close as it is to real-world clinical practices that include recruiting participants, setting, design, delivery and execution of interventions, determination and analysis results, as well as primary analyses. This is a major difference between explanatory trials as defined by Schwartz & Lellouch1, which are designed to prove the hypothesis in a more thorough way.
The trials that are truly pragmatic should not attempt to blind participants or clinicians as this could cause bias in estimates of the effect of treatment. Pragmatic trials should also seek to attract patients from a wide range of health care settings, to ensure that the results can be applied to the real world.
Furthermore the focus of pragmatic trials should be on outcomes that are crucial to patients, like quality of life or functional recovery. This is especially important for trials that involve the use of invasive procedures or could have harmful adverse consequences. The CRASH trial29, for example focused on the functional outcome to compare a two-page report with an electronic system to monitor the health of patients in hospitals suffering from chronic heart failure. In addition, the catheter trial28 utilized urinary tract infections that are symptomatic of catheters as its primary outcome.
In addition to these aspects, pragmatic trials should minimize the requirements for data collection and trial procedures to reduce costs and time commitments. Furthermore pragmatic trials should strive to make their findings as applicable to clinical practice as is possible by making sure that their primary method of analysis follows the intention-to treat approach (as described in CONSORT extensions for pragmatic trials).
Despite these criteria, many RCTs with features that defy pragmatism have been incorrectly self-labeled pragmatic and published in journals of all kinds. This could lead to false claims about pragmatism, and the term's use should be made more uniform. The creation of the PRECIS-2 tool, which offers an objective and standard assessment of pragmatic characteristics, is a good first step.
Methods
In a practical trial the goal is to inform clinical or policy decisions by showing how an intervention could be incorporated into real-world routine care. This is distinct from explanation trials that test hypotheses about the cause-effect relationship in idealised conditions. Therefore, pragmatic trials might be less reliable than explanatory trials and might be more susceptible to bias in their design, conduct and analysis. Despite their limitations, pragmatic research can provide valuable data for making decisions within the context of healthcare.
The PRECIS-2 tool evaluates the degree of pragmatism in an RCT by assessing it across 9 domains ranging from 1 (very explanatory) to 5 (very pragmatic). In this study, the recruitment, organization, flexibility in delivery, flexible adherence and follow-up domains scored high scores, but the primary outcome and the method of missing data were not at the limit of practicality. This indicates that a trial can be designed with effective practical features, but without harming the quality of the trial.
It is, however, difficult to judge how practical a particular trial is, since pragmatism is not a binary characteristic; certain aspects of a trial can be more pragmatic than others. Moreover, protocol or logistic changes during the trial may alter its score in pragmatism. Additionally 36% of the 89 pragmatic trials identified by Koppenaal and colleagues were placebo-controlled or conducted before licensing and most were single-center. They are not in line with the norm, and can only be referred to as pragmatic if their sponsors agree that the trials are not blinded.

Results

For instance, the right type of heterogeneity can help the trial to apply its results to many different settings and patients. However the wrong type of heterogeneity may reduce the assay's sensitivity, and thus lessen the ability of a study to detect small treatment effects.
A number of studies have attempted to classify pragmatic trials with various definitions and scoring systems. Schwartz and Lellouch1 created an approach to distinguish between explanation-based trials that support the clinical or physiological hypothesis as well as pragmatic trials that inform the choice of appropriate therapies in the real-world clinical setting. The framework consisted of nine domains that were evaluated on a scale of 1-5 which indicated that 1 was more explanatory while 5 being more pragmatic. The domains included recruitment, setting up, delivery of intervention, flex compliance and primary analysis.
The original PRECIS tool3 was based on a similar scale and domains. Koppenaal and colleagues10 created an adaptation of the assessment, called the Pragmascope, that was easier to use for systematic reviews. They found that pragmatic systematic reviews had higher average scores in the majority of domains but lower scores in the primary analysis domain.
The difference in the primary analysis domain could be explained by the fact that the majority of pragmatic trials analyze their data in an intention to treat manner while some explanation trials do not. The overall score for systematic reviews that were pragmatic was lower when the areas of organisation, flexible delivery and following-up were combined.
It is crucial to keep in mind that a pragmatic study should not mean a low-quality trial. In fact, there is increasing numbers of clinical trials which use the term "pragmatic" either in their abstracts or titles (as defined by MEDLINE but which is not precise nor sensitive). These terms may indicate an increased understanding of pragmatism in abstracts and titles, but it's unclear if this is reflected in the content.
Conclusions
As the value of evidence from the real world becomes more widespread and pragmatic trials have gained traction in research. They are randomized trials that compare real world alternatives to clinical trials in development. They include patient populations that are more similar to those who receive treatment in regular care. This method has the potential to overcome the limitations of observational studies, such as the biases that arise from relying on volunteers, and the limited availability and the variability of coding in national registry systems.
Other advantages of pragmatic trials include the ability to use existing data sources, as well as a higher likelihood of detecting meaningful changes than traditional trials. However, these tests could be prone to limitations that undermine their validity and generalizability. For example, participation rates in some trials may be lower than anticipated due to the healthy-volunteer effect and incentives to pay or compete for participants from other research studies (e.g. industry trials). Practical trials are often restricted by the need to recruit participants in a timely manner. Some pragmatic trials also lack controls to ensure that observed differences aren't caused by biases during the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-described themselves as pragmatist and published up to 2022. They assessed pragmatism using the PRECIS-2 tool, which includes the domains eligibility criteria and recruitment criteria, as well as flexibility in intervention adherence, and follow-up. They discovered that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or above) in at least one of these domains.
Trials with high pragmatism scores tend to have more criteria for eligibility than traditional RCTs. They also contain populations from many different hospitals. The authors claim that these characteristics can help make the pragmatic trials more relevant and applicable to everyday practice, but they do not necessarily guarantee that a trial using a pragmatic approach is completely free of bias. The pragmatism characteristic is not a fixed attribute and a test that doesn't have all the characteristics of an explicative study could still yield valid and useful outcomes.
